Patients are hereby informed that compounded medications are not reviewed, evaluated, or approved by the U.S. Food and Drug Administration (FDA) for safety, effectiveness, or quality. These medications are custom-prepared in accordance with a valid prescription and are regulated by relevant state and federal laws governing pharmacy compounding practices. However, they are not subject to FDA approval or clinical trials.

Despite these precautions, compounded medications may carry risks that differ from those associated with FDA-approved drugs, such as potential variability in strength, purity, stability, and effectiveness.

By choosing to use a compounded medication, the patient acknowledges and accepts these risks, and agrees that the use of the medication is undertaken with informed consent and at the patient’s own risk.

Patients are encouraged to address any questions, concerns, or potential risks about compounded medications directly with their prescribing healthcare provider or the dispensing pharmacy.
